Listen, then tap the pitches in order. Easy and Medium are crotchets only — pitch order is enough. Hard also scores rhythm, including beamed quavers.

Yes. A new melody is issued once per UTC day for each difficulty, and everyone who plays that day gets the same sequence, so you can compare results with friends without spoiling the notes.
